1.      Explain your Agency's understanding of its role within Georgia's child welfare system?

 

2.      Does your Agency have an operational understanding of child welfare programming?  Describe how your agency operates for each of the programs you are approved to provide.

 

3.      Provide a brief explanation of your Agency's community and organizational development?

 

4.      Provide a brief description of your facilities as they relate to your programming goals?

 

5.      Does the community support your Agency?  Explain?

 

6.      Does your Agency have external accountability in place for the organization and program?  Please describe.

 

7.      Provide a brief explanation of your philosophy of caring and nurturing children?

 

8.      Describe the makeup and credentials of your staff.

 

9.      Does your agency have a mission statement?  Do you also have a document that explains the ethical and legal standards by which your agency operates?  Please attach copies of each.

 

10.  Is your agency fiscally sound?  Provide an overview of how you generate your major sources (by percentage) of revenue?  RBWO, Charitable Donations, Corporate Support, DJJ, Medicaid, etc.

 

11.  Do you have any outstanding citations from the Office of Regulatory Services?  If yes, please explain.

 

12.  Is your agency under investigation for any reason by the Office of Investigative Services?

 

13.  Have you received any programming violations or citations within the past 12 months?  If yes, please explain.
